How have you been, and what is the energy like currently at LJ Multispecialty Hospitals?
Dr Manesh Agarwal: Thank you so much for the incredibly warm welcome! It is a true pleasure to sit down with TheCconnects. I have been doing exceptionally well, though my days are always deeply engaged. The energy at LJ Multispecialty Hospitals right now is electric. We are in the thick of commissioning our 250-bed facility. We are building this institution to be future-ready from day one, meaning we are heavily focused on implementing AI-assisted clinical care and intelligent operations. It is a demanding phase, but building a system from the ground up that prioritizes scalable, patient-centric care is incredibly fulfilling.
TheCconnects: Your transition from a practicing clinician to a top-tier executive is fascinating. Can you tell our readers a little about your professional journey and how you came to your current role?
Dr Manesh Agarwal: My journey began in the clinical trenches. I started as an ICU Registrar at Bombay Hospital and Medical Research Centre. Working in the ICU, I realized very early on that while a doctor can save one patient at a time, a highly optimized healthcare system can save thousands. I wanted to be the person fixing the system.
That realization led me to pursue my M.Phil in Hospital Administration and Health Care Management from BITS Pilani. I transitioned from clinical practice into administration, eventually serving as the Senior OSD to the Medical Director at Bombay Hospital in Mumbai. Over the years, I took on the mandate of turning around and scaling major hospitals across the country. I led the commissioning of a 300-bed greenfield hospital for Narayana Hrudayalaya in Jaipur, served as VP Corporate at Apollo Health City in Hyderabad, and later as CEO Dr. Jivraj Mehta Hospital in Ahmedabad. Today, as Group CEO at LJ Multispecialty Hospitals, I am merging my 25 years of operational expertise with my passion for AI and digital transformation.
TheCconnects: Leadership is often shaped by profound moments and mentors. Who has influenced you the most in life and why?
Dr Manesh Agarwal: While I have been guided by many visionary medical directors, my greatest influence has always been the patients and the frontline staff. I strongly believe that leadership is not something you only see in boardrooms you see it in the hospital corridors.
There was a defining moment during a hospital round a few years ago. I noticed an elderly patient who had been waiting for hours to get admitted. I immediately intervened to start his treatment, but I didn’t stop there. I folded my hands and apologized to him on behalf of my entire team for the delay in his treatment. The apology itself wasn’t the focal point, the impact on the hospital culture was. The staff saw that the most senior person in the hospital was not afraid to take accountability. That experience reinforced my core philosophy: leadership does not just change policies; it changes culture. If my apology restores a patient’s trust, I will do it without hesitation, but my actual job is to build a system where that delay never happens again.
TheCconnects: You are well known for executing massive hospital turnarounds. What are the biggest challenges you have faced in these executive roles, and how did you overcome them?
Dr Manesh Agarwal: The biggest challenge in hospital administration is rarely medical. It is almost always operational and cultural. When I took over as CEO at Dr. Jivraj Mehta Hospital, the goal was to transform a legacy 200-bed institution into a digitally enabled, patient-first organization. We faced significant challenges regarding cost control, legacy workflows, and resistance to digital adoption.
I overcame this by instilling absolute transparency and a results-driven culture. We introduced a full-time consultant model to ensure care continuity, which drastically improved patient satisfaction. We aggressively pursued NABH accreditations across all key departments, making quality an everyday habit rather than an annual audit. By strictly monitoring KPIs and taking the entire hospital completely paperless, we managed to improve our EBITDA margins from negative 6% to positive 10% and boosted bed occupancy by 25%. You overcome cultural resistance by showing the team that efficiency actually makes their daily jobs easier.
TheCconnects: Those are incredible metrics. What key lessons have you learned from your professional career about building sustainable healthcare systems?
Dr Manesh Agarwal: The most profound lesson I have learned is that the greatest opportunity in any organization lives inside its processes. You can hire the best doctors in the world, but if your billing is delayed, your pharmacy is mismanaged, or your nursing staff is overwhelmed with paperwork, the patient experience will still fail.
Another key lesson is the necessity of digital literacy. I pursued a PG Diploma in Medical Law, Policy & Ethics recently to better understand clinical ethics and patient rights in the digital age. Healthcare leaders must realize that technology is not a replacement for empathy, it is a tool that removes the friction from operations so that doctors and nurses have more time to actually be empathetic.
TheCconnects: Today, every industry is racing toward automation. What do you see as the biggest challenge for healthcare brands in the digital space?
Dr Manesh Agarwal: The biggest challenge is the chaotic implementation of technology. Most healthcare leaders recognize that AI is the future, but they struggle with how to bring it to life strategically. Often, hospitals purchase expensive software or AI tools and force them onto a staff that is not adequately trained. This creates fatigue and operational bottlenecks. The challenge in the digital space is moving beyond the buzzwords to actual, seamless adoption where the entire team is energized and on board.
TheCconnects: You are highly regarded as an AI & Automation Strategist. How does your specific framework address these exact pain points for hospitals and business leaders?
Dr Manesh Agarwal: To address the chaos of technology implementation, I developed an AI adoption framework called the “Circle of Confidence.” I apply this framework to ensure that when we introduce workflow automation or AI-assisted diagnostics, we do it in a way that builds trust among the clinical staff.
Because I have led from the CEO chair and practiced from the clinician’s chair, I understand exactly what both sides need. My solutions focus on making processes intelligent and scalable. For example, by automating revenue cycle management and digitizing pathology lab systems, we eliminate manual errors and administrative bloat. This framework addresses the pain points by ensuring that AI implementation is not disruptive; instead, it accelerates outcomes, frees up teams to do their best work, and makes growth highly sustainable.
TheCconnects: Between managing hospitals, speaking at TEDx, and consulting, your schedule must be intense. What do you do in your free time?
Dr. Manesh Agarwal: Free time is scarce, but I try to use it to create a broader impact. I love writing and sharing my experiences, which led to the publication of my book, “Visionary Leadership Redefined,” recently launched by the eminent actress Padmini Kolhapure. I also spend time engaging with other industry leaders through platforms like OPSCON and HOPECON, serving as a chairperson or jury member to discuss the future of healthcare. On a personal note, I prioritize fitness. During the COVID-19 lockdown, I actually participated in a fitness challenge and achieved a High Range Book of World Records recognition for performing the most knee strikes in one minute to honor frontline warriors! It keeps me disciplined and energized.
TheCconnects: That is brilliant and highly inspiring! Finally, do you have any advice for aspiring entrepreneurs, doctors, and young leaders looking to make an impact in the healthcare industry?
Dr. Manesh Agarwal: My advice to young doctors and healthcare entrepreneurs is to look beyond the clinical chair. India is expanding its healthcare infrastructure rapidly, but we have a massive shortage of people who actually understand how to run these complex systems. If a doctor can treat a disease, a healthcare leader can cure the hospital. Learn about finance, embrace AI, understand quality accreditations, and prioritize ethics. Build systems that are so robust that they can deliver excellent care to thousands of people simultaneously. Do not just build a practice; build a scalable legacy.
